2014-2015 Annual Membership Dues (AMD) and Data Update
Complete this online form to remit your annual membership dues to the Chartered Professional Accountants of Ontario
(registered name of The Institute of Chartered Accountants of Ontario)("CPA Ontario") and any amount charged to CPA Ontario by
the Chartered Professional Accountants of Canada (CPA Canada/CICA) on behalf of the Member. The membership dues are aligned with
CPA Ontario's fiscal year end and cover the new fiscal period April 1, 2014 to March 31, 2015.
Dues Remittance Changes:
Membership dues are based upon data on file at the time of billing. Should your membership dues billing code need to be changed,
please ensure that you have made all the required changes to your membership data. All requests for changes to the membership dues billing code
will be reviewed and must be supported by updated membership data.
Transfer To Another Provincial Accounting Body:
Many Members choose to retain memberships with more than one provincial accounting body. There is a reduced fee for the non-primary membership
which may be obtained by amending your billing code in the dues remittance change section. If you choose not to retain membership with
CPA Ontario you will need to complete the Membership Resignation Form 4-3A.
Members who are providing public accounting or public accounting services to the public are reminded of the requirement to register a practice.

Continuing Professional Development (CPD) Declaration:
Each year, in conjunction with the annual membership dues process, all Members must submit a CPD Declaration Form to CPA Ontario.
Non-retired or newly retired Members complete CPD Declaration Form 4-5A/D declaring whether they met or are exempt from the CPD requirement.
Retired Members complete CPD Declaration Form 4-5F, declaring that they continue to meet the CPD retired exemption. Members who indicate that
they did not comply with the CPD requirement MUST also submit a CPD Plan of Action Form 4-5B. Only one plan of action will be accepted in
each three-year CPD cycle.
Public Accounting Licence (PAL):
Members, who hold a Public Accounting Licence (PAL) or may be applying for a PAL, must complete the annual minimum CPD requirements of 20 hours,
including at least 10 hours of verifiable CPD. The CPD must be completed in activities directly related to the competencies needed to provide
public accounting services. Exemptions from the 2013 CPD requirements or failure to comply with the annual minimum
CPD requirements impact your ability to renew or apply for a public accounting licence.
All information and documentation required by this application MUST be submitted in full by the Member no later than June 1, 2014
without a late payment or June 30, 2014 with a late payment. Members, who fail to pay the dues, provide all information and
produce all documents and other materials as requested by the Registrar or, in extraordinary circumstances where such documentation is not available,
provide alternate proof satisfactory to the Registrar, by the June 30, 2014 deadline may be suspended.
